A person-to-person session always yields the best results, but when you are too busy or live in an area that makes it difficult to come into Liza’s office, then have the next best thing… have Liza come to your home via a custom recording, made especially for you. Simply copy and fill out our detailed questionnaire and send it in with your investment payment of $125.00 (or via paypal). Please fill out the form carefully and with as much detail as possible. The recording will be made specifically for you with your name and your particular issues — just as if you were in the office for a session! Please be sure to include your phone number, best shipping address, your email address and best time to contact you in case any additional information is required and to advise you when your custom recording is complete and on its way. (form will be sent to you upon receipt of full payment).

Telephonic Hypnosis Session

By signing below, I (we) “Client(s)” hereby accept(s) services to be provided by HealWithin/InnerSite, Inc., for on-line/Telephonic recorded Hypnosis sessions under the following terms and conditions:

To make a Telephonic Recorded Hypnosis session with Liza, it is best recommended to call 48 hours in advance to schedule and reserve your appointment. To make sure you will receive full benefit of your session, please allow at least two hours of undisturbed time away from interruption such as someone walking in on you, pets jumping on you or cell phones ringing.

One hundred ninety five dollars ($195) per session-up to one and half hours, and a $75 charge for each additional half hour thereafter.

• This shall include a personalized recording of each session via CD by Liza Boubari in accordance to your session.
• Appointment cancellations shall be accepted up to 6 hours before scheduled appointment time without incurring a charge. Last minute cancellations (less than 6 hours) are subject to $120 charge.
• HealWithin/InnerSite, Inc. makes no claims as to benefits – medical, physical, or otherwise-for services provided.
• Client agrees to hold Liza Boubari, HealWithin/InnerSite harmless in the event of any unforseen result arising directly or indirectly from services provided. Client personally assumes all risks.
• Client agrees to pay for any and all fees and costs related to any legal action brought by Client against HealWithin/InnerSite, Inc., and further agrees to resolve disputes through arbitration according to the laws of the State of California.

The Client has read and agrees to the terms herein. ________ initial.
